Participant information form for the Online Survey

Parents and carers completing the online survey are not required to sign and return the participant information form, please simply keep a copy of it for your records.

If you are participating in the Interface Jammers activity, your child will need to watch the participant information video first. After your child has watched the video, please check that they consent to participating before you sign the form.
